1. ACKNOWLEDGMENT OF RISK
I understand that participation in Muay Thai, kickboxing, boxing, martial arts instruction, sparring, grappling, strength and conditioning, fitness training, pad work, bag work, drills, competitions, seminars, open mat sessions, and all related activities involves inherent risks.
These risks include, but are not limited to:
·Cuts and bruises
·Sprains and strains
·Broken bones
·Concussions and other head injuries
·Neck or spinal injuries
·Joint injuries
·Dental injuries
·Eye injuries
·Internal injuries
·Permanent disability
·Paralysis
·Death
I understand these injuries may result from my own actions, the actions of other participants, equipment failure, or the ordinary negligence of released parties.
I knowingly and voluntarily choose to participate despite these risks.
2. ASSUMPTION OF RISK
I voluntarily assume all known and unknown risks associated with participation at Reno Muay Thai.
I accept full responsibility for any injury, illness, disability, property damage, or death arising from participation.
3. RELEASE OF LIABILITY
To the fullest extent permitted by Nevada law, I release, waive, discharge, and covenant not to sue:
·Reno Muay Thai
·Zach Bunnell
·All owners
·Employees
·Coaches
·Volunteers
·Independent contractors
·Training partners
·Students
·Guest instructors
·Event organizers
·Property owners
·Landlords
·Sponsors
·Any affiliated individuals or entities
from any and all claims, demands, causes of action, damages, liabilities, costs, attorney’s fees, or expenses arising out of or related to my participation, including claims alleging ordinary negligence.
This release does not apply where prohibited by law.
4. COVENANT NOT TO SUE
I agree that I will not file or participate in any lawsuit or legal claim against any released party for injuries or damages covered by this agreement.
5. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless the released parties from any claims, lawsuits, liabilities, judgments, attorney’s fees, or expenses arising from my participation or my actions while attending Reno Muay Thai.
6. MEDICAL FITNESS
I certify that:
·I am physically able to participate.
·I have consulted a physician if necessary.
·I have disclosed any medical conditions that could affect my participation.
·I understand Reno Muay Thai is not providing medical advice.
I agree to stop participating immediately if I experience symptoms that make continued participation unsafe.
7. MEDICAL TREATMENT
If I become injured or require emergency medical treatment, I authorize Reno Muay Thai to obtain emergency medical care on my behalf if I am unable to do so.
I understand I am solely responsible for all medical expenses.
8. EQUIPMENT
I agree to use protective equipment when required, including gloves, shin guards, mouthguard, headgear when required, groin protection, and any other equipment required by instructors.
I understand that protective equipment reduces—but does not eliminate—the risk of injury.
9. RULES OF THE GYM
I agree to:
·Follow all instructor directions.
·Treat training partners respectfully.
·Train in a controlled manner.
·Immediately stop when instructed.
·Report injuries promptly.
·Maintain personal hygiene.
·Wear appropriate training equipment.
·Refrain from training while under the influence of alcohol or drugs.
Failure to follow these rules may result in suspension or termination of membership without refund.
10. PERSONAL PROPERTY
I understand Reno Muay Thai is not responsible for lost, stolen, or damaged personal property.
11. PHOTO AND VIDEO RELEASE (OPTIONAL)
I authorize Reno Muay Thai to photograph or record me during classes, seminars, competitions, or events.
These images or recordings may be used for advertising, websites, social media, promotional materials, or educational purposes without compensation.

12. SEVERABILITY
If any portion of this agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.
13. ENTIRE AGREEMENT
This document constitutes the entire agreement regarding liability and supersedes any prior oral or written representations.
14. GOVERNING LAW
This agreement shall be governed by the laws of the State of Nevada.
Any legal action relating to this agreement shall be brought in the appropriate state or federal court located in Washoe County, Nevada, to the extent permitted by law.
15. ACKNOWLEDGMENT

·I understand this agreement is intended to be as broad and inclusive as permitted by Nevada law.
